Advantage Partner Structure
All members must sign a contract committing to pay the fees for a full 12 months either monthly, quarterly, or up front via cash, check or credit card.

- One yearly comprehensive physical exam.
- All office visits during regular office hours included.
- All home visits from 7am to 9pm are included, whether to patient's home or office.
- All questions or requests via phone or email 24/7.
- Priority scheduling for same day office visits.
- File every visit actually done on insurance for patient.
- Laceration treatment including suturing during office hours.
- Any lab offered in house: UA, EKG, Wet Prep, Cholestech Lipid Panel.
Prices
- Member: $125/month, $500/quarter, $1500/year
- Family of 2 (Member and Spouse or Dependent): $240/month, $960/quarter, $2,880/year
- Family of 3 (Member and Spouse and/or Dependents): $375/month, $1,440/quarter, $4,320/year
- Family of 4 (Member and Spouse and/or Dependents): $500/month, $1,920/quarter, $5,760/year
- Family of 5 (Member and Spouse and/or Dependents): $625/month, $2,400/quarter, $7,200/year
* Extras:
- House calls or office visits between 9pm and 7am will be $50 extra each visit.
- Any lab that is sent to Quest Diagnostics, but partner gets to take advantage of excellent pricing (or we can bill labs to insurance).
- All radiology/X-Rays, but partner gets to take advantage of excellent pricing (or imaging can be billed to insurance).
